Rol of Serum Angiopoietin-2 and Von Willebrand Factor in Renal Affection Syrian Patients with Systemic Lupus Erythematosus

Abstract

Background: Several lines of evidence demonstrate excessive endothelial activation in systemic lupus erythematosus (SLE). Angiopoietin-2 (Ang-2) and Von Willebrand Factor are two biomarkers of endothelial cell activation that may be clinically useful in SLE.

Objective: to examine the clinical utility of two proposed biomarkers of endothelial activation; angiopoietin-2 and Von Willebrand Factor in SLE correlating their serum concentration with disease activity and renal affection.

Subjects and methods: Ang-2 and Von Willebrand Factor were measured in sera obtained from 60 patients with SLE (6 male patients and 54 female) and 30 healthy controls by ELISA methodology. Disease activity was calculated according to the SLE Disease Activity Index (SLEDAI). Patients were classified according to disease activity and renal involvement.

Results: Both Ang-2 and WF concentrations were increased in patients with SLE in comparison to controls (P<0.001). Both Ang-2 and WF were significantly higher in the active group and active nephritis subgroup in comparison to inactive group and active patients without nephritis, respectively (P<0.001). Both markers were positively correlated with SLEDAI and protein/ creatinine ratio. Serum Ang-2 was superior to serum WF as predictor of SLE activity and discriminator of lupus nephritis.

Conclusion: We can Rely on Elevated concentrations of circulating Von Willebrand Factor and Ang-2 as biomarker of disease activity and renal affection in SLE. We found that Serum Ang-2 was superior to serum Von Willebrand Factor as predictor of SLE activity and nephritis. These Results should be conducted to put serum Ang-2 as a candidate drug target where functional blockade could seem to protect against endothelial damage in such disease.

دور الأنجوبويتين-2 (ANG-2) المصلي وعامل فون ويلبراند Von Willebrand Factor في تطور العلة الكلوية لدى مرضى الذآب الحمامي السوريين

الخلفية العلمية أـثبتت العديد من الدراسات وأشارت إلى العلاقة الوطيدة ما بين الأذية البطانية ومرض الذآب الحمامي الجهازي. يعد  Angiopoietin-2  وVon Willebrand Factor two  واصمان مهمان في تفعيل الخلية البطانية مما قد يجعله مفيد سريرياً فيSLE.  

هدف الدراسة فحص الاستخدامات السريرية المتعددة للواصمين الحيويين المقترحين للتفعيل البطاني وهما الانجيوبيوتين-2 وعامل Von Willebrand Factorعند الأشخاص المصابين بمرض الذئبة الحمامية الجهازية وكذلك تحديد التراكبز المصلية لكلا الواصمان والمرتبطة بالعلة الكلوبة لهذا المرض.

المواد والطرق حددت التراكيز المصلية لكل من  Ang-2و WF في مصول 60 مريضاً مصاباً بمرض الذئبة الحمامية الجهازية) SLE 6 ذكور و54 إناث) و30 شواهد أصحاء بطريقة وفق منهجية الإليزا enzyme-linked immunosorbent assay (ELISA).

النتائج كانت نسب كل من الأنجيوبويتين وعامل Von Willebrand Factorتزداد بالمقارنة مع الأصحاء (P<0.001), كما سُجِلَت مستويات مرتفعة لكلا الواصمين عند مجموعة الأشخاص المصابين بالذئبة الحمامية النشطة والمجموعة الفرعية للأشخاص المصابين بالتهاب الكلية.

أظهر كل من الواصمين ارتباط إيجابي مع (SLEDAI) ومعدل بروتين/ كرياتينين،  كما أن مستويات الأنجيوبويتين- 2 المصلية  كانت أعلى من مستويات عامل Von Willebrand Factor المصلية كمؤشر لنشاط مرض الذئبة الحمامية الجهازية وممِيز لالتهاب الكلية المصاحبة لهذا المرض.

الاستنتاجات يمكن الاعتماد على التراكيز المرتفعة من عامل فون ويلبراند والأنجيوبويتين-2 في جهاز الدوران كواصمَين حيويَّين لتحديد نشاط مرض الذآب الحمامي الجهازي والعّلة الكلوية المرتبطة به، وجدنا أن مستويات الأنجيوبويتين- 2 المصلية كانت أعلى  من مستويات عامل فون ويلبراند المصلية لذلك تعتد كمؤشر لنشاط مرض الذئبة الحمامية الجهازية والالتهاب الكلوي، حيث تبين هذه النتائج إمكانية استخدام مصل الأنجيوبويتين- 2 كهدف دوائي مرشح وظيفته الحماية ضد الأذية البطانية لهذا المرض.
