دراسة مقارنة بين التثبيت الرقبي الأمامي مع صفيحة أو بدونها في فتق النواة اللبية الرقبي مع اعتلال جذري
Abstract
هدف هذه الدراسة هو تحليل الفروقات في النتائج السريرية والشعاعية لاستئصال فتق النواة اللبية الرقبي والتثبيت الأمامي مع وبدون صفيحة رقبية أمامية قافلة.
تم تحليل النتائج ل54 حالة أجريت لهم الجراحة بين 2015 و 2017 , حيث قسموا إلى مجموعتين رئيسيتين , والفرق الأساسي بين المجموعتين في التقنية هو إضافة صفيحة رقبية أمامية , وكانت استطبابات الجراحة لكلا المجموعتين متماثلة.
وقد حاولنا دراسة الالتحام العظمي والنتائج السريرية بين المجموعتين , وأهم ما وجدناه هو أن إضافة صفيحة رقبية أمامية تقلل من عدد حالات النتاج السريري السيء. فقد أدى التثبيت الإضافي بصفيحة رقبية أمامية قافلة إلى معدلات التحام عظمي أعلى إلى تحسن في الألم الرقبي المحوري أفضل مما وجدناه في مجموعة المرضى دون صفيحة , وخاصة عندما يتم التداخل على ثلاث مستويات أو أكثر.
The aim of this study is to analyse the differences in clinical and radiological outcome of anterior cervical discectomy and fusion for cervical degenerative disease, with and without the addition of an anterior cervical locking plate.
The outcome of 54 cases operated between 2015 to 2017 were analysed, and divided into two groups. The main difference in technique between the two groups was the addition of an anterior cervical plate. The indications for surgery for both groups was identical.
We made an attempt to study radiological fusion, clinical outcome between the non-plated and plated groups. Our main finding is that the addition of an anterior plate reduces the number of poor clinical outcomes. Anterior discectomy and fusion with plating In our series had a significantly higher fusion rate, and better axial neck pain outcome compared to the non-plating group, especialy when operating on three or more cervical levels.
