Abstract

تأثير الرضوض الصوتية على حاسة السمع وتدبيرها

هدف الدراسة معرفة تأثير الرضوض الصوتية على حاسة السمع وكيفية متابعتها وعلاجها سواء جراحياً أو دوائياً حسب نمط الإصابة, ومدى تحسن السمع بعد العلاج عند هؤلاء المرضى.

الطرق: شملت عينة الدراسة 50 مريضا تراوحت أعمارهن بين 18 –55 سنة و امتدت فترة الدراسة لمدة عام من شهر نيسان 2017 – نيسان 2018, حيث تم متابعة المرضى المراجعين للعيادة الاذنية في مشفى تشرين الجامعي بشكاية أذنية تالية للتعرض لرض صوتي وبفترات مختلفة من التعرض, حيث تم فحص المرضى سريريا مع إجراء تنظير أذن بالمنظار وإجراء تخطيط سمع لجميع المرضى لمعرفة نوع نقص السمع الحاصل ثم تقديم العلاج المناسب ( دوائي أو/ و جراحي ), تم تقييم نتيجة العلاج حسب معطيات الفحص السريري ونتائج التخطيط السمعي.

متغيرات الدراسة: تم تقسيم المرضى حسب نمط نقص السمع الحاصل بعد الرض الصوتي, العمر, وقت البدء بالعلاج بعد التعرض للرض الصوتي, ونتيجة العلاج .

النتائج: 3 مرضى (6%) كان لديهم نقص سمع توصيلي و23 مريض (46%) كان لديهم نقص سمع مختلط, 24 مريض (48%) كان لديهم نقص سمع حسي عصبي, وسجلت لدينا النتائج التالية:

لوحظ وجود علاقة بين نمط نقص السمع الحاصل والتحسن بعد العلاج حيث أن نقص السمع التوصيلي أو المختلط تحسن بعد الجراحة بشكل ملحوظ, أما نقص السمع الحسي العصبي المتأخر بالعلاج لم يتحسن على العلاج واحتاجوا معينات سمعية, 26 مريض تعرضوا للرضوض الحادة الانفجارية عولجوا جراحيا (23 مريض تحسنوا بعد العلاج الجراحي, 3 مرضى تم علاجهم جراحيا فقط لحماية الأذن من الوسط الخارجي واحتاجوا معينات سمعية ),24 مريض تم علاجهم دوائيا ( 4 مرضى تعرضوا لرض ضجيجي تم تحسنهم بعد العلاج المبكر خلال 48 ساعة من التعرض للرض, 20 مريض ممن تعرضوا للرض الضجيجي المزمن وراجعوا بعد عدة أشهر لسنوات من التعرض للرض لم يتحسن السمع لديهم واحتاجوا معينات سمعية ).

الخلاصة: الرض الصوتي باختلاف مسبباته يسبب أنماط مختلفة من نقص السمع يختلف علاجها وتحسنها. فالرض الضجيجي المزمن المترافق مع التطور الصناعي يعتبر من أهم المسببات لنقص السمع غير المستجيب للعلاج والتي يمكن تجنبه بالعلاج المبكر بعد التعرض للرض, حماية الأذن بالواقيات عند المعرضين للرضوض خاصة عمال المصانع .

The aim of study is to determine the effect of acoustic trauma on hearing and how to follow up and manage these cases whether surgically or medically according to the type of injury , and the degree of hearing improvement after treatment in these patients.

Methods : The study involved 50 patients between 18-55 years old ,over a period of one year between April  2017 to April 2018 .

We visited patients coming to ENT clinic in Tishreen University Hospital who had complaints related to being exposed to acoustic trauma , both acute or noise chronic trauma.

We performed clinical examination for the patient using Otoscope , then the patients underwent pure tone audiometry to determine the type of hearing loss  , then we treated accordingly whether medically or surgical treatment as needed .

We evaluated the result of our treatment using clinical examination and pure tone audiometry .

Variables :  We divided the patients depending on ( type of hearing loss after trauma , age, starting time  of treatment after trauma  , and treatment results ).

Results : 3 patients (6%) had conductive hearing loss.  23 patients (46%) had mixed hearing loss. 24 patients ( 48%) had sensorineural hearing loss. We noticed a relationship between type of hearing loss and the improvement in hearing after treatment :

Conductive and mixed  types of hearing loss got remarkable improvement while sensorineural hearing loss didn’t get better and patients needed hearing aids.

  • 26 patients who had acute acoustic trauma were treated surgically ( 23 had improved  after surgery while 3 patients were only operated just to protect the ear and they needed hearing aids )
  • 24 patients were treated medically ( 4 patients with acoustic trauma got better after early treatment within 48 hours after trauma ).   And (20 patients who had chronic noise trauma and were seen  after months or years of trauma didn’t get better and needed hearing aids ).

Conclusion :Acoustic trauma regardless of its cause can result in different types of hearing loss with different outcomes after treatment. Chronic acoustic trauma these days accompanying industrial development is considered to be one of the main causes of irresponsive to treatment hearing loss, which can be avoided by early treatment , and by wearing protection ear plugs in factory workers.
