TY - JOUR AU - عبد الله, نادر AU - حسن, يانا PY - 2018/11/19 Y2 - 2024/03/28 TI - دراسة مدى تأثير تاريخ بدء المعالجة الكيميائية بعد الجراحة على معدل البقيا دون نكس عند مرضى سرطان الثدي غير النقائلي JF - Tishreen University Journal -Medical Sciences Series JA - Tuj-hlth VL - 40 IS - 5 SE - Articles DO - UR - https://journal.tishreen.edu.sy/index.php/hlthscnc/article/view/4715 SP - AB - <p>يملك العلاج الكيميائي دوراً أساسياً في تدبير سرطان الثدي. ولا يوجد حتى الآن أي دليل حول التوقيت المثالي لبدء المعالجة الكيميائية. لذلك أجرينا هذه الدراسة الاسترجاعية لتقييم تأثير توقيت بدء المعالجة الكيميائية على معدل البقيا بدون نكس، وذلك عند مرضى سرطان الثدي غير النقائلي في قسم الأورام في مشفى تشرين الجامعي، اللاذقية، سوريا. شملت الدراسة عينة من مريضات سرطان ثدي (من عام 2010 حتى عام 2013) بمراحل (I - III)، معالجة بالجراحة والمعالجة الكيميائية. وقد تم جمع المعلومات الإنذارية المتعلقة بالعمر، حجم الورم، النمط النسيجي للورم، الدرجة النسيجية، إصابة العقد اللمفية، حالة المستقبلات الهرمونية وحالة مستقبل عامل النمو الظهاري -2. تم توثيق الفاصل الزمني بين الجراحة وبدء المعالجة الكيميائية، توقيت حدوث النكس، وتوقيت الوفاة. وتم تصنيف المرضى إلى 3 مجموعات وفقاً للفاصل الزمني بين الجراحة وبدء العلاج الكيميائي: &gt;4 أسابيع (المجموعة الأولى)، 4 – 8 أسابيع (المجموعة الثانية)، &lt; 8 أسابيع (المحمودة الثالثة).  <strong> </strong></p> <p>شملت الدراسة 299 مريضة، شكّل السرطان القنوي الغازي (90%) من مريضات البحث، فيما السرطان الفصيصي الغازي بقية الحالات (10%). خضع 93.6% من مريضات البحث لاستئصال ثدي تام وأجري في 6.4% استئصال ثدي جزئي. شكّل سرطان الثدي إيجابي الأستروجين غالبية مريضات البحث بنسبة 75.5%، وشكّل سرطان الثدي إيجابي Her2 نسبة 32.3%.  لم يكن هنالك فرق في (5-year OS) وفقاً لتوقيت بدء العلاج الكيميائي المعزز. يترافق البدء المتأخر للعلاج الكيميائي (&lt; 8 أسابيع مقابل &gt;4 أسابيع) وكذلك البدء المتوسط (4 -8 أسابيع مقابل &gt;4 أسابيع) مع تناقصٍ هامٍ في (5-year DFS) (HR of 2.7; 95% CI:1.7 – 4.2) وHR of 1.61 ; 95% CI: 1.18 – 2.2)) على الترتيب.</p> <p>قمنا أيضاً بدراسة تأثير توقيت البدء بالعلاج الكيميائي على الإنذار لدى أنماط الورم حسب حالة المستقبلات الورمية. ترافق البدء الباكر للعلاج الكيميائي (&gt; 4 أسابيع) مع (5-year DFS) و (5-year OS) أفضل عند مريضات سرطان الثدي من مختلف الأنماط الورمية بالمقارنة مع البدء خلال (4-8 أسابيع) (P&gt;0.05) والبدء المتأخر (&lt; 8 أسابيع) (P&lt;0.05) باستثناء النمط (ER-/Her2).</p> <p>The adjuvant chemotherapy in breast cancer has an established role in the management. However, there is no evidence about the optimum time of initiation of treatment. So this retrospective study was<strong> </strong>to evaluate the influence of timing of initiation of the adjuvant chemotherapy over survival in non-metastatic breast cancer<strong> </strong>at Oncology Department, Tishreen university Hospital in Lattakia, Syria. The study enrolled breast cancer patients (from 2010 to 2013) with stages I–III, treated with surgery and adjuvant chemotherapy. Data were gathered about prognostic factors such as age, tumor size, histological grade, lymph nodes involvement, hormonal receptor status and human epidermal growth factor 2. The time interval between surgery and initiation of chemotherapy, and dates of relapse and death were recorded. Patients were assigned in three groups according to the surgery-chemotherapy interval: &lt; 4 weeks, 4-8 weeks, &gt;8 weeks.</p> <p><strong>The</strong> study enrolled 299 patients, with a mean age of 53.6 years (age range: 32 – 82 years). IDC was the most common breast cancer type (90%), the rest of cases were ILC (10%).  93.6% of study patients had mastectomy, 6.4% had partial breast surgery. Positive estrogen receptor breast cancer represented the most cases (75.5%). 32.3% of cases were Her2 positive.  There were no differences in 5-year overall survival (OS), according to the timing of initiation of adjuvant chemotherapy. Longer delay of initiation of adjuvant chemotherapy (&lt;4 weeks versus &gt;8 weeks) and moderate delay (&lt;4 weeks versus 4-8 weeks) significantly decreased the DFS (HR of 2.7; 95% CI,1.7 – 4.2) and (HR of 1.61; 95% CI, 1.18 – 2.2) respectively.</p> <p>We further investigated the effect of time to adjuvant chemotherapy on disease free survival according to subtypes. Earlier initiation of adjuvant chemotherapy was associated with better5 – year DFS in patients with ER+/Her2- and ER-/Her2+ tumors.</p> ER -
