مقارنة البروبوفول مع التيوبنتال/هالوتان لتخدير العمليات قصيرة الأمد عند الأطفال

Abstract

أجريت الدراسة في قسم التخدير والإنعاش في مشفى الأسد الجامعي في عام 2009, وشملت 100 طفل تراوحت أعمارهم بين 3-13 عاماً أجريت لهم عمليات جراحية قصيرة مثل: استئصال لوزات وناميات (38%)، عمليات على العضلات العينية (2%)، عمليات بولية تناسلية (60%).

قسم المرضى إلى مجموعتين :

الأولى : خضع المرضى فيها للبروبوفول, حيث تم استخدامه في المباشرة الوريدية وفي الاستمرارية تسريباً وريدياً مستمراً.

الثانية : استخدم فيها التيوبنتال للمباشرة وكانت الاستمرارية باستخدام الهالوتان .

هدفت الدراسة إلى دراسة خصائص المباشرة التخديرية و فترة الصحو من حيث نوعية المباشرة والاختلاطات وسرعة الإنباب والصحو والتخريج من قسم العمليات  .

أما نتيجة الدراسة فكانت: أن نسبة حدوث توقف التنفس، الفواق، تشنج الحنجرة، الغثيان والإقياء عند أطفال المجموعة الأولى أقل مما هي عليه عند أطفال المجموعة الثانية, وذلك في كل من مرحلتي المباشرة والصحو، كما أن الإنباب والتخريج من قسم العمليات تم خلال فترة زمنية أقصر في المجموعة الأولى.

والخلاصة: أن استخدام البروبوفول لمباشرة واستمرارية التخدير هي تقنية تخديرية جيدة التحمل عند الأطفال ونسبة اختلاطات قليلة في مرحلتي المباشرة والصحو وتؤمن انباباً سريعاً وتخريجاً سريعاً من قسم العمليات, خاصة في العمليات الجراحية القصيرة.

This study was  carried out in the Department of Anesthesia and Resuscitation at  AL-Assad Univresity Hospital, Lattakia, Syria, in 2009 and included 100 children aged between 3 and 13 years. The most common surgical procedures performed were adeno-tenseloctomy (38%), eye muscle surgery (2%), and genitourinary surgery (60%). Patients were divided into two groups. Propofol was used in the first group for induction and maintenance by infusion, while Thiopental was used in the second for induction and Halothane for maintenance. The aim of this study was to evaluate the quality of induction and recovery periods, the complications during induction and recovery, and the speed of extubation and discharging from the operation section. The result was that the percentage of breathlessness, hiccup, laryngeal spasm, and nausea and vomiting in children of first group was less than it was in the second during the two phases of induction and recovery. Also, the extubation and discharging from the operation section took place in a shorter time than the first group. The conclusion was that the use of propofol for induction and maintanence of anesthesia was a well-tolerated anesthesic technique in children; it had less complications in induction and recovery, providing quick extubation and quick discharging from the operation section, especially in short term surgeries.
