Effect of intravenous oxytocin infusion on blood loss during myomectomy

Abstract

Background: Open myomectomy remains the main option has been shown to have satisfactory results as regard fertility preservation and symptom resolution, especially when the number and size of leiomyomas do not permit any other routes of intervention. Hemorrhage remains the most important concern with open myomectomy. Blood loss during surgery mainly depends on the number, size of the removed fibroids and the technique adopted during myomectomy.

Purpose: The aims of this study are to evaluate the effectiveness of oxytocin infusion during abdominal myomectomy on blood loss. In addition to, to determine the safety of oxytocin use.

Patients and Methods: This is randomized clinical study conducted for the period one year (July 2020 –July 2021) at Tishreen University Hospital in Lattakia-Syria. The study included 80 women who underwent myomectomy. They were assigned randomly either to group I (40) who received oxytocin or to group II (40) who received saline.

Results:  The mean age was 33.7±4.4 years, without significant difference between the two groups regarding demographic characteristics, and ultrasonographic findings of the uterus and myoma(p>0.05). The mean blood loss in group I was lower (390±159.1) vs (663.75±215.1, p:0.0001) in group II. Hemoglobin and hematocrit decline was more evident in the group II (1.95±0.9 vs 0.91±0.4, p: 0.0001) and (4.26±1.4 vs1.80±0.8, p:0.0001) respectively. The need for blood transfusion was 5% in group I vs 27% in group II, p:0.006. The duration of operation and hospitalization was significantly longer in group II (97.75±14.1 vs 72.12±10.5, p:0.0001) and (1.67±0.7 vs1.15±0.3, p: 0.002) respectively. Oxytocin infusion was associated negatively with blood loss (r: -0.31, p:0.001).

Conclusion: Oxytocin infusion is an effective and safe method in reducing bleeding during myomectomy.
