دراسة مقارنة للتطعيم الحرقفي بين الطعوم القطعية وطعوم الرقائق العظمية الصغيرة (2012-2013)
Abstract
من المشاكل التي قد تواجه الجراحين تغير حجم الطعوم الحرقفية المستعملة في تطعيم الفقدان العظمي في العظم السنخي وذلك بسبب الامتصاص الذي يحدث في هذه الطعوم خلال شفائها وتحولها إلى عظم بشكل كامل .
هدفت هذه الدراسة لاجراء مقارنة بين الطعوم المأخوذة من الحرقفة على شكل قطعة عظمية
block bone graft بواسطة السنابل الاسطوانية المجوفة Trephine وبين الطعوم العظمية التي يتم الحصول عليها بواسطة المكاشط العظمية bone scraper ، حيث تألفت عينة الدراسة من 10 مرضى من الذكور من المراجعين لمشفى جراحة الوجه والفم و الفكين في كلية طب الأسنان جامعة دمشق في الفترة الزمنية بين 2010-2012 م ، وكان المرضى يعانون من ضمور واضح في مناطق الارتفاعات السنخية العلوية والسفلية لأسباب عديدة وبحاجة لتطعيم عظمي لتعويض هذا الفقدان وتحقيق المكان المناسب لاستقبال الزرعات السنية .لوحظ تحقق الالتحام العظمي في الطعوم العظمية الحرقفية وتحولها بشكل كامل إلى عظم طبيعي، ولكن كانت الطعوم الحرقفية القطعية محافظة على حجمها بشكل أكبر من الطعوم العظمية المأخوذة بالمكشط العظمي . استعمل اختبار Mann-Whitney U والبرنامج الحاسوبي SPSS لتحليل البيانات ، حيث أظهرت النتائج أنه عند إعادة الترميم العظمي لمناطق الضمور باستعمال الطعوم الحرقفية القطعية فإن هذه الطعوم لم تتعرض للامتصاص كما هو الحال عند استعمال الطعوم العظمية المأخوذة بالمكشط العظمي ، ولكنها كانت تتعرض إلى الانكشاف وتراجع اللثة حول حواف الطعم بشكل تالٍ للعمل الجراحي مع أفضلية واضحة للطعوم المأخوذة بالمكاشط العظمية عليها في هذا الجانب .
The common problem that may encounter surgeons were the change of the volume of iliac bone grafts which used in atrophic region in the alveolar bone because of the resorption of this grafts during healing period.
This study aimed to compare between block graft which taken by surgical bur (trephine)and grafts which collected by bone scraper ,in this way the bone graft look like small chips .This study included 10 patients who presented the oral and maxillofacial department in dentistry faculty in Damascus university between 2010-2012 A.D. the patients suffered from sever atrophy in upper and lower alveolar bone for different underlying causes and needed to compromise these atrophy to create a suitable area for dental implantation .The iliac bone grafts were turned into a real bone in the grafted area but the block grafts were saving their volumes more than the grafts collected by bone scraper. We used Mann-Whitney U and SPSS to evaluate the data that collected during the research and the results revealed that the block iliac bone graft which used to compromise the atrophic region would resorb less than the grafts which collected by bone scraper . But these grafts showed that it would be more exposed after surgery than the bone scraper grafts .
