تأثيرات زيادة جرعة المباشرة من السيزأتراكوريوم على زمن بدء تأثيره والحصار الباقي في وحدة الإنعاش

Abstract

تهدف هذه الدراسة إلى تقييم زيادة جرعة المباشرة من السيزاتراكوريوم , المرخي العضلي غير النازع للاستقطاب والذي يملك مدة تأثير متوسطة , ودراسة تأثيرها في زمن البدء وظروف التنبيب الرغامي والحصار الباقي في وحدة الإنعاش وإمكانية تطبيق ذلك في ممارستنا الطبية .

تم إدخال خمسين مريضاً في هذه الدراسة العشوائية المستقبلية وقسموا إلى مجموعتين متساويتين : مجموعة A بجرعة 0.15 مغ/كغ ومجموعة  Bبجرعة0.25  مغ/كغ من أجل مباشرة التخدير .لم تستخدم مثبطات الكولين استراز في المعاكسة وتمت مراقبة سحب الأنبوب الرغامي والحصار الباقي في وحدة الإنعاش باستخدام حاث العصب المحيطي بطريقة قطار الأربعة . كانت نتائج الحصول على ظروف تنبيب ممتازة في المجموعة B في مدة أقصر من دون تطاول مدة التأثير السريري ولم تلاحظ زيادة معتبرة في حالات الحصار الباقي في وحدة الإنعاش .

يمكن أن نشجع على زيادة جرعة المباشرة من السيزأتراكوريوم ولكن يجب أن نستخدم حاث العصب المحيطي في مراقبة الوظيفة العضلية العصبية خاصة عندما يتم الصحو من دون استخدام مثبطات الكولين استراز  .

 

This study aims to evaluate the increase of induction dose of cisatracurium, a non- depolarising neuromuscular blocker which has an intermediate duration of action, and to study its effect on the onset time, the intubating conditions , the clinical duration and residual block in the recovery unit and the possibility of applying this in our medical practice.

We enrolled fifty patients in this randomized prospective study and divided them into two equal groups: Group A  (0.15mg/kg) , group B (0.25mg/kg) for induction of anesthesia . Cholinesterase inhibitors were not used for antagonism; extubation and residual block were monitored by using peripheral nerve stimulator, the method of train-of-four.

As a result , excellent intubating conditions were achieved in group B in a shorter time without prolonging the clinical duration. We did not find a considerable increase in cases of residual block in the recovery unit .We can encourage increasing the induction dose of cisatracurium , but we should use the peripheral nerve stimulator in monitoring the neuromuscular function especially when we achieve the recovery without cholinesterase inhibitors.
