معالجة التهاب الجيوب الفكية المزمن المترافق مع ضمور في الغشاء المخاطي لجوف الأنف
Abstract
تم معالجة 47 مريضا" ممن يشكون من التهاب مزمن في الجيوب الفكية مع ضمور على مستوى الغشاء المخاطي لجوف الأنف, بالإضافة إلى مجموعة ثانية ضمت 36 مريضا" ممن يشكون من التهاب مزمن في الجيوب الأنفية بدون ضمور في الغشاء المخاطي لجوف الأنف, وذلك خلال الفترة بين 1998-2002 , وقد تبين من خلال المعالجة الكلاسيكية لإلتهاب الجيوب الفكية المزمن أن هناك نكسا" كبيرا" في الأعراض السريرية عند المجموعة الأولى التي يترافق فيها الإتتهاب مع ضمور في الغشاء المخاطي لجوف الأنف.ومن خلال متابعة الفحوص الدموية لمرضى المجموعة الأولى تبين وجود نقص في عيار الحديد في المصل مع ارتفاع معدل سعة الرابط للحديد , وعند استخدام مركبات الحديد في المعالجة انخفض معدل النكس وتحسنت الأعراض السريرية , وحدث الشفاء التام .
من خلال ما تقدم نجد كلما كان نقص عيار الحديد في المصل عاليا" كلما كان الضمور في مخاطية الأنف شديدا" لذا يجب معايرة الحديد عند مرضى التهاب الجيوب الفكية المترافق مع ضمور في الغشاء المخاطي لجوف الأنف وإضافته إلى العلاج موضعيا" او بالطريق العام .
It wase treated 47 patients with chronic maxillary sinusitis and associated atrophy of nasal cavity mucosa. And 36 patients with chronic maxillary sinusitis alone.The patients of the first group were found to have subclinical iron deficieny .The treatment involved intranasal infusions of ( vasalin+ lonaline+ ferrum) ointment –impregnated tampons introduced into the nasal cavity.Derangement of iron metabolism is consideredto be responsible for poor response rate and recurrences.It suggested that iron preparations be included in treatment for chronic maxillary sinnsitis with nasal mucosa atrophy.
