دور تنظير الصدر بمساعدة الفيديو في تشخيص أسباب انصباب الجنب

Abstract

تشخيص أمراض الجنب هي واحدة من المشاكل الطبية المتكررة. وعلى الرغم من توفر مجموعة متنوعة من الاختبارات التشخيصية لتقييم الانصباب الجنبي, لاتزال ( 15-20٪) من الانصبابات الجنبية دون تشخيص, يأتي دور الجراحة التنظيرية هنا جذرياً وحاسماً بحيث تسمح بدراسة عيانية مباشرة لكامل جوف الجنب، وأخذ خزعات كافية من المناطق المشتبهه. الهدف من الدراسة هو تقييم فعالية وسلامة تنظير الصدر بمساعدة الفيديو في تشخيص أسباب الانصبابات الجنبية . أجرينا خلال 3 سنوات (حزيران 2012 - حزيران 2015 م ) 38 تنظيرًا صدرياً بقصد تشخيص انصبابات الجنب الغامضة-(التي لم يُعرف سببها رغم جميع الاستقصاءات غير الباضعة بما فيها بزل و خزعة الجنب عبر الجلد)- وأخذنا خزعات موجهة منها جميعاً. شكلت الآفات الخبيثة في خزعات الجنب (60,5%) ، والتدرن (31,6%) ، وجاءت خزعة الجنب التهابًا لا نوعيًا في باقي الخزعات (7,9%) . تضمنت الآفات الخبيثة: ميزوتليوما (60,9%)، نقائل لسرطانة غدية (30,4%) و لمفوما لا هودجكن (8,7 %). لم تحدث أي وفاة لدى مرضانا، و ظهرت أربع مضاعفات . الخلاصة: تنظير الصدر بمساعدة الفيديو هو إجراء آمن ودقيق لتشخيص ومعالجة أمراض الجنب وعلاوة على ذلك فإنه يوفر الوقت والجهد والتكلفة، كما يمكن للمريض أن يتعافى ويعود إلى نشاطه الطبيعي بسرعة. The diagnosis of pleural diseases is one of the frequent clinical problems. Despite a variety of diagnostic tests are available for evaluating pleural effusions, approximately 15–20% of pleural effusions remains undiagnosed, In this cases ,a thoracoscopy plays an important role, it permits the surgeon to visually view and evaluate the entire pleural space ,and to obtain enough accurate biopsies from suspected areas. This study aims to evaluate the effectivenes of Video Assisted Thoracoscopic surgery (VATS) in diagnosis of pleural effusions. Between June 2012 and June 2015 we performed 38 diagnostic thorososcopy for undiagnosed pleural effusions. The histopathologic studies of pleural biopsies result were as follows : (60,5%)malignancies, (31,6%) TB And (7,9%) nonspecific inflammatory reaction. The malignant diseases are: Mesothelioma (60,9%), Metastases of adenocarcinoma (30,4%),Lymphoma (8,7 %) 4 Complications ,with no mortality. Conclusion: Video Assisted Thoracoscopic Surgery is a safe and accurate diagnostic procedure for pleural diseases but it also has a very useful role in therapy.(VATS) saves time, effort and cost, more over it is preferred by the patient as he recovers and returns to his normal activity rapidly
