تقييم نتائج التهوية الآلية في قسم العناية المشددة في مشفى الأسد الجامعي
Abstract
قمنا بدراسة حول تطبيق التهوية الآلية في قسم العناية المشددة بعد دراسة سابقة له منذ 6أعوام لنتبين ما وصلنا إليه في تطبيق هذا العلم والتركيز على النقاط الإيجابية والإضاءة على المواقع السلبية بطريقة العمل.
أجرينا البحث في قسم العناية المشددة في مشفى الأسد الجامعي في اللاذقية على مدى عامين متتاليين
2005-2006 بلغ حجم العينة 185مريضاً لجأنا لتطبيق التهوية الآلية لديهم, درسنا توزع المرضى حسب الفئات العمرية والاستطبابات والاختلاطات, ودرسنا نسبة الفطام والوفيات وأسبابها. وكانت نتائج البحث:
a) نجح الفطام بنسبة 9 .51 % وكانت نسبة الوفيات1. 48 %.
b) أهم أسباب الفطام الناجح كانت قلة الاختلاطات واختيار الوقت والطريقة المناسبة للفطام والمتابعة الجيدة والمستمرة للمريض وأيضاً تعاون الأهل معنا.
c) أعلى نسب الفطام كانت للغرق والأمراض الصدرية, وللفئات العمرية الأقل ولمرضى التهوية قصيرة الأمد, وبالتأكيد للمرضى من دون اختلاطات.
d) بالمقارنة مع الدراسات المحلية السابقة والدراسات العالمية المذكورة نتأكد بتقارب النتائج من المستوى العالي للأداء الطبي المقدم لمرضانا.
This study of The Mechanical Ventilation Treatment (M.V.T) in the Intensive Care Unit (I.C.U) at Al-assad University Hospital comes after 6 years of similar one, trying to assess our experience, reinforce the positive points and get rid of the negative ones. The study was done over a period of two years 2005-2006.185 patients were ventilated. The study included: age, indications, complications, weaning rate and causes of death.
Results were:
- the rate of weaning was 51.9% and the mortality was 48.1%;
- the successful weaning was because of complications, good choice of time and method, continuous follow-up, and family cooperation;
- topmost weaning rates were drowning and chest disease ones, small ages, and patients without complications;
- comparison with international and native previous studies. We can watch high medical work given to our patients by the convergence of results.
