ترشيد استخدام السالبوتامول الاستنشاقي في أمراض الطرق التنفسية الإنسدادية خبرة مشفى الأسد الجامعي
Abstract
أجريت دراسة على 300 مريض ممن راجعوا العيادة الصدرية الخارجية في مشفى الأسد الجامعي بين عامي 2008-2009 ، وهم مرضى ربو قصبي أوCOPD ومرضى نفاخ رئوي مشخص سابقاً. أظهرت هذه الدراسة المبالغة الكبيرة وغير المبررة في استخدام السالبوتامول الاستنشاقي على المدى الطويل. 100مريض (33,12%) استخدموا أكثر من 50 بخة في الأسبوع و28 مريض (5،%8) أكثر من 110بخات في الأسبوع. 139 مريض (46%) لم يحصل لديهم أي تحسن بقيم PEF بعد الاستنشاق, من هؤلاء 97 مريض (33 ( %32, لم يشعروا بتحسن سريري ولم تتحسن قيم PEF لديهم، و 42 من المرضى ( 14 % ) أصابهم تحسن سريري لكن دون تحسن بقيم PEF ، غالبية هؤلاء المرضى كانوا ممن يستخدمون جرعات عالية تزيد عن الخمسين بخة في الأسبوع. مشاركة الكورتيزونات الاستنشاقية زاد من فعالية السالبوتامول أما الاستخدام الخاطئ للمستنشق فزاد من عدد البخات المستخدمة أسبوعيا . الأعراض الجانبية للعلاج بالسالبوتامول الاستنشاقي كانت قليلة وأهمها الرجفان وتسرع القلب. إن تحليل هذه النتائج يدفع إلى التوصية بعدم استخدام أكثر من عشر بخات سالبوتامول أسبوعيا على المدى الطويل، وتستدعي الحاجة إلى أكثر من هذه الجرعات إعادة التقيم من قبل الأخصائي بالأمراض التنفسية والمشاركة مع أدوية أخرى وبصفة خاصةً الكورتيزونات ( الستروئيدات ااقشرية ) الاستنشاقية.
This study included 300 asthma and COPD patients admitted to the pulmonary clinics at Al– Assad University Hospital in Lattakia during 2008 – 2009. This study showed the increasing and unjustified use of salbutamol inhaler in long- term treatment. 100 patients (33%) used more than 100 puffs per week and 28 patients (8.5%) used more than 110 puffs per week. 139 ( 46% ) patients did not show any improvement (PEF) after inhalation. Of these patients, 97 (32.33%) improved neither clinically nor functionally, and 42 patients (14%) improved clinically, but without functional improvement (PEF). The majority of these patients used high doses (more than 50 puffs per week). Additional steroid inhaler increased the effectiveness of Salbutamol, while misusing the inhaler increased the number of puffs used weekly; the side-effects of Salbutamol inhaler were low, and the most important were tremors and tachycardia. The analysis of these results led to the recommendation not to use more than 10 puffs of Salbutamol inhaler a week; patients should be reevaluated by a pulmonary specialist for adding other medications, especially steroid inhalers.
