تقييم وظائف الرئة عند مرضى قصور الدرق
Abstract
Aim of the study: The main objective of this study is to study the effect of hypothyroidism on lung function values and to indicate the pattern of injury and the relationship between the degree of disability and these values.
Methods: A comparative cross-sectional study of 80 adult from endocrinology outpatients department and other internal clinics between March 2018 and February 2019. Patients with diabetes, smokers, pregnant women, obstructive and restrictive pulmonary patient and BMI> 30 were excluded. The study included two comparison groups. The first sample included patients with hypothyroidism with TSH> 4 mL/L versus the control sample with TSH (0.04-4) mL/l. The FT4 laboratory values for both groups were taken and the pulmonary function test (PFT).
Results: Both comparison groups were similar in age and sex, and body mass index (BMI) was higher in patients with hypothyroidism P-value (0.002). There was a decrease in FVC, FEV1, FEF25-75% in patients with hypothyroidism with statistically significant values P- value (0.001-0,002-0.04) respectively, while there was no significant decrease in values PEF, FEV1/FVC% with P-value (0.5-0.05). TSH significantly negatively correlated with FEV1 and FVC while they significantly positively correlated with FT4.
Conclusion: Hypothyroidism affects respiratory function and leads to lower lung function values.
الهدف: إن الهدف الأساسي لهذا البحث هو دراسة تأثير قصور الدرق على قيم وظائف الرئة وتبيان نمط الإصابة والعلاقة بين درجة القصور وهذه القيم.
المرضى والطرق: دراسة مقطعية مقارنة شملت (80) مريضاً بالغاً من مراجعي العيادة الغدية والعيادات الداخلية الأخرى في الفترة الممتدة بين آذار (2018) وشباط (2019)، تم استبعاد مرضى السكري والمدخنين والحوامل والمرضى مع دلائل لمرض رئوي ساد أو حاصر والمرضى مع مشعر كتلة الجسم (BMI)30<. ضمت الدراسة مجموعتي مقارنة، شملت العينة الأولى مرضى قصور الدرق مع TSH>4 ميللي وحدة/ل مقابل عينة الشاهد مع سواء درقي TSH (0.04-4) ميللي وحدة/ل وتم أخذ القيم المخبرية للـ FT4 لكلا المجموعتين وإجراء اختبار وظائف الرئة بمقياس التنفس البسيط.
النتائج: كانت كلا المجوعتين متماثلتين بالعمر والجنس، أمّا مشعر كتلة الجسم (BMI) فكان أعلى عند مرضى قصور الدرق P-value (0.002). تبين حدوث انخفاض بقيم (25-75%)FEF، FEV1، FVC عند مرضى قصور الدرق مع قيم مهمة إحصائياً P-value (0.04، 0.002، 0.001) على التوالي، في حين لم يكن هناك انخفاض مهم بقيم الـ PEF، (%)FVC/FEV1 مع P-value (0.05-0.5). وبدراسة العلاقة بين قيم الـTSH وFT4 وقيم الـ FEV1، FVC تبين وجود علاقة سلبية بين TSHوهذه القيم مقابل علاقة إيجابية بينها وبين الـ FT4.
الاستنتاج: قصور الدرق يؤثر على الوظيفة التنفسية ويؤدي إلى انخفاض قيم وظائف الرئة.
