معدل حدوث سرطان الموثة في مستوياتPSA بين4 – 20 نانوغرام /مل
Abstract
أجريت دراسة استقصائية على 115مريضاَ من مراجعي مشفى الأسد الجامعي باللاذقية في الفترة الواقعة بين 12/2009 و 12/2010 حيث تمت معايرة المستضد الموثي النوعي عند المراجعين لمشفى الأسد في قسم الجراحة أو العيادات الخارجية والذين تجاوزت أعمارهم الخمسين عاماً وهم يعانون من الأعراض البولية السفلية LUTS وذلك بهدف الكشف المبكر عن سرطان الموثة .
تم تقسيم المرضى حسب نسبة PSA في المصل إلى مجموعتين : الأولى نسبة PSA 4-10 نانوغرام/مل و الثانية 11-20 نانوغرام/ مل ومن ثم أخذت خزعة للموثة حتى مع عدم احتمال وجود اشتباه بالمس الشرجي, ودراسة نتائج الخزع, وكانت النتائج كالتالي: نسبة سرطان الموثة في المجموعة الأولى 12% ( 8 مرضى من أصل 68 مريضاً ) , وفي المجموعة الثانية كانت نسبة سرطان الموثة 23% عند ( 11 مريضاً من أصل 47 مريضاً).
إن هذه الدراسة تؤكد ضرورة الاهتمام الجدي بالمرضى ذوي PSA أكبر من 4 نانوغرام /مل وتضع بين أيدينا احتمالية هذه الاصابة السرطانية عندما تكون نسب PSA بهذا المستوى , وبالتالي إجراء دراسات مسحية موسعة عند جميع الذكور فوق الخمسين عاماً لمعايرة المستضد الموثي النوعي بشكل روتيني.
This study was conducted on 115 patients in the University Al-Assad Hospital, in Lattakia, during the period from 12/2009—9/2010. The prostatic specific antigen was assayed in patients in Surgery Department or in the clinics of Al-Assad Hospital; their age is above 50 years; and suffer from Low Urinary Tract Symptoms (LUTS) in order to early detect their Prostate Cancer.
The patients were divided according to PSA levels into two groups. The first PSA level 4-10 nanogram/m. The second PSA level 11-20 nanogram/ml. Then the patients undergone a prostatic biopsy even without suspect by DRE. The results were as follows: the detection rate in first group was 12% (8 out of 68 patients), and in the second group was 23% (11 out of 47 patients).
This study assures the important care in patients who have PSA levels above 4 nanogram/ml and consider the prostate cancer in these patients. So it suggests to make screening studies for men above 50 years old by assaying PSA routinely .
