مشعر جديد لتقييم حالة الجنين أثناء المخاض

Abstract

تضمنت الدراسة /300/ تخطيط لقلب الجنين أثناء المخاض لمواخض عشوائيات، أجري لهنّ دراسة تحليلية شاملة حسب كل مكون على حدة، وفي المجمل بمشعري كريبس، والمشعر المعدّل الجديد، ومقارنة كل ذلك مع الوسط الغازي لدم الشريان السري، وأبغار الوليد.

لقد توصلنا إلى النتائج التالية: إن مكونات تخطيط قلب الجنين لها دور مهم في التنبؤ بحالة الجنين وكانت
( تسرع وتباطؤ قلب الجنين المديدين، غياب التغيرية والتسارعات القصيرة ، وجود تسارعات لانموذجية، ظهور تباطؤات قصيرة لا نموذجية متحولة، و تباطؤات متأخرة )

1- إن نوعية كلا المشعرين كريبس، و المعدّل الجديد كانت عالية (94%  , 92%)على التوالي.

2- إن القيمة التنبؤية السلبية للمشعرين كريبس، والمعدّل الجديد كذلك كانت عالية (89%, 82%) على التوالي.

3-إن الحساسية منخفضة حسب مشعر كريبس 32%) )، و وصلت إلى أكثر من الضعفين حسب المشعر المعدّل الجديد (69%).

4- إن القيمة التنبؤية الإيجابية لمشعر كريبس (36%) و (78%) للمشعر المعدّل الجديد.

يجب الاعتماد على المشعر المعدّل الجديد في تخمين حالة الجنين، وبالتالي التقليل من نسبة التداخلات الجراحية ( قيصرية – ملقط – محجم ) بأكثر من الضعفين  2.2/ /

Three hundred intrapartum C.T.G were analyzed and compared with fetal arterial cord blood acid base and Apgar score.

The fetal heart rate attribute that was found to be predictive of neonatal outcome was: tachycardia, bradycardia, absence of variability and accelerations, presence of atypical accelerations, severe late decelerations and atypical variable decelerations.

The evaluations of fetal state by Krebs and the new modified score were as follows:

  1. Specificity of  two scores was high ( 92%, 94%) respectively.
  2. Sensitivity of  two scores was (32% , 69%) respectively.
  3. The predictive value of the positive test was (36% , 78%) respectively.
  4. The predictive value of the negative test was ( 82%,89%) respectively

Evaluation of the fetal state during labour via the new modified score gives a high sensitivity and a high predictive value of positive test; this means operative intervention will decreased by 2,2 folds.
