دراسة الكسب العظمي الذروي والامتصاص العظمي الحفافي في الزرع السني التالي لرفع الجيب الفكي المغلق مع أو بدون استخدام الطعم العظمي الذاتي - دراسة مقارنة-
Abstract
تقدم الدراسة تقنية حديثة توسع دائرة استطباب الزرع السني لتشمل مرضى لا يتوفر لديهم عظم كاف في المنطقة الخلفية للفك العلوي وتغني عن تقنيات أخرى أكثر رضاً ، وتهدف إلى المقارنة بين استخدام وعدم استخدام الطعم العظمي الذاتي في الزرع السني التالي لإجراء رفع الجيب الفكي المغلق من حيث تقييم الكسب العظمي الذروي والامتصاص العظمي الحفافي، وقد شملت الدراسة 22 زرعة سنية نوع advance وضعت لدى 10مرضى (5 ذكور، 5إناث) تراوحت أعمارهم (34-63) سنة ، كل المرضى لا يعانون من أمراض عامة أو موضعية ، وغير مدخنين ، ويستطب لديهم الزرع مع رفع الجيب الفكي المغلق في المنطقة الخلفية للفك العلوي، وقسمت الزرعات إلى مجموعتين :
A : 11 زرعة سنية وضعت بدون طعم عظمي ذاتي .
B : 11 زرعة سنية وضعت مع طعم عظمي ذاتي .
و كانت نتائج الدراسة الشعاعية للصور البانورامية الرقمية المسجّلة بعد 6 أشهر (قبل التحميل تماماً) على النحو الآتي:
متوسط الكسب العظمي الذروي 3.2 ملم في المجموعة A و3.8 ملم في المجموعة B ، متوسط الامتصاص العظمي الحفافي 1.1 ملم في المجموعة A ، و 0.5 ملم في المجموعة B ، ومما تقدم نوصي بتطبيق الرفع المغلق للجيب الفكي لوضع الزرعة السنية بالطول المناسب مع استخدام الطعم العظمي الذاتي لتحسين الكسب العظمي الذروي وتقليل الامتصاص العظمي الحفافي .
This current study offers a modern technique that widens indication for dental implantation in posterior area of maxilla and substitutes other more harmful techniques. It aims to compare between using and not using autogenous bone graft in implants inserted after closed sinus lift according to apical bone's gain and marginal bone's resorption. This study involves 22 (advance's type) implants inserted in 10 patients (5 male, 5 female). All patients are healthy, not smokers, and have indication for implantation with closed sinus lift in posterior area of maxilla. Implants divided into two groups:
A: 11 implants inserted without autogenous bone graft
B: 11 implants inserted with autogenous bone graft
The results of digital radiographic panoramic study after 6 months (actually before loading) were that:
Rate of apical bone's gain was 3.2 mm in group A and 3.8 mm in group B, rate of marginal bon's resorption was 1.1 mm in group A, and 0.5 mm records in group B.
So, we recommend applying closed sinus lift to insert implants with suitable length and using autogenous bone graft to improve apical bone's gain and decrease marginal bone's resorption.
