تقييم فعالية البلازما الغنية بالصفيحات الدموية على الشفاء العظمي بوجود طعم فوسفات الكالسيوم
Abstract
هدفت هذه الدراسة إلى معرفة مدى تأثير البلازما الغنية بالصفيحات PRP في التجدد والشفاء العظمي بعد عملية استئصال الأكياس الفكية، و ذلك من خلال دراسة التغيرات السريرية والشعاعية من حيث الكثافة العظمية . تألفت عينة البحث من 18 مريضاً و مريضة ( 14 ذكراً و 4 إناث ) كانت لديهم أكياس متوسطة القطر تتراوح
بين 1 – 3 سنتمترات في الفك العلوي، بلغ عددها لدى المرضى الخاضعين للبحث 20 كيساً . قسمت هذه العينة إلى مجموعتين متساويتين :
- المجموعة التجريبية: طبقت عليها البلازما الغنية بالصفيحات PRP مع طعم ثلاثي فوسفات الكالسيوم .
- المجموعة الشاهدة: طبق عليها طعم ثلاثي فوسفات الكالسيوم من دون وجود البلازما الغنية بالصفيحات PRP .
تمت مراقبة المرضى سريرياً وشعاعياً ، حيث جرت المراقبة الشعاعية من خلال الصور الشعاعية البانورامية الرقمية قبل العمل الجراحي وبعده مباشرة، وبعد 3 أشهر و من ثم بعد 6 أشهر و ذلك من أجل دراسة الكثافة العظمية من خلال برنامج ديجورا Digora الحاسوبي . لقد تبين من خلال الدراسة الإحصائية لنتائج المراقبة السريرية والشعاعية لحالات البحث أنه يوجد فروقات دالة إحصائياً من حيث مشعر الكثافة العظمية خلال فترات المراقبة بينما كان مشعر الألم و الانتان غير دالين إحصائياً . نستنتج من ذلك أن استخدام البلازما الغنية بالصفيحات يسرع من عملية الشفاء العظمي في سياق ترميم العيوب العظمية الناجمة عن استئصال الأكياس الفكية بعد 3 و 6 أشهر تالية للعمل الجراحي .
This study aims to know the effect of PRP on bone healing and regeneration after cystoectomy within studying clinical and radiological bone density changes. The research sample comprised 18 patients (14 males, 4 females) (20 cystic lesions in the upper jaw,1-3 cm diameter). This sample was divided into 2 similar groups :
- The experimental group: Platelets-rich Plasma with Tri-Calcium Phosphate was applied.
- The control group: only Tri-Calcium Phosphate, without Platelets-rich Plasma, was applied.
Both groups were observed clinically and radiologically. According to radiological observation, the digital panoramic radiographs were taken before, immediately after surgery, after 3 months, and after 6 months, respectively. Bone density measurements were done through Digora® digital program.
The present study showed that there is no statistical difference between the experimental group & the control group, regarding Pain Index and Infection Index; while there was a statistical difference between the experimental group & the control group, regarding bone density after 3 and 6 months after surgery.
Within the limitations of the present study, the application of autologous platelets-rich plasma (PRP) accelerates bone regeneration after cystoectomy .
