مقارنة بعض عناصر الثبات في تحضير الأسنان بين طلاب جامعتي دمشق وتشرين
Abstract
تعدُّ درجة ميلان الجدران المحورية من العوامل الهامة التي تؤثر في ثبات التعويضات الثابتة في الحفرة الفموية.ويتأثر هذا الثبات بعدد من العوامل أهمها: درجة تقارب الجدران المحورية وارتفاعها, وشكل الحدود العنقية ونوع الإسمنت.
قيمت هذه الدراسة درجة ميلان الجدران المحورية للدعامات السنية التي حضّرت لاستقبال التعويضات الثابتة في عيادات قسم التيجان و الجسور في جامعة دمشق و تشرين بمعدل 150 و 136 حالة على التوالي.
ووجد بأن أقل قيمة زاوية تحضير قد بلغت في الأسنان الأمامية العلوية 8 في جامعة دمشق و8.5 في جامعة تشرين أما عن زوايا التحضير في كافة الأسنان في الجامعتين فكان يزيد أكثر من 10 درجات عن المعايير المقبولة عالميا. أما من ناحية الشكل الهندسي للسطح الطاحن فكان ممسوح المعالم في جامعة تشرين 62.04% بينما في دمشق 8.04% وقليلة الوضوح في تشرين 25.93% وفي دمشق 35.71% واضح المعالم في جامعة تشرين 12.03% وفي دمشق 56.25%. أما من ناحية الحدود العنقية فنجد أنه في جامعة تشرين بلغت حالات شبه الكتف 76.37% وحالات خط الإنهاء البسيط 23.63% بينما بلغت حالات شبه الكتف في جامعة دمشق 30.67%
وحالات خط الإنهاء البسيط 69.33% .
نلاحظ كبر ميلان الجدران في الجامعتين مقارنة بالحدود القصوى التي أوصت بها الدراسات العالمية الكثيرة في هذا المجال والذي بطبيعة الحال سوف يؤثر بدرجة كبيرة في الثبات، ناهيك عن اختلاف الخطة العلاجية بين الجامعتين والاهتمام بمعالم السطح الطاحن ونوع الحدود العنقية وبالتالي فإنه سيؤثر أيضاً في نجاح التعويض الثابت واستقراره ومدة بقاء هذا التعويض في فم المريض.
The aim of this study was to evaluate and compare the taper degree of prepared teeth between Damascus university and Tishreen university. The samples consist of 150 stone cast of prepared teeth from the fixed prosthodontic clinic at Damascus university, and 136 stone cast of prepared teeth from the fixed prosthodontic clinic at Tishreen university.
The mean value of taper degree of Damascus university samples was 8 D whereas the value of samples of Tishreen university was 8.5 D. The anatomic features of occlusal surface and finish line were also more detectable in Damascus university samples.
Although the treatment plan may be different at both universities but our study recommended the importance of taper degree, occlusion surface, and finishing line of teeth preparation to provide retention and success of fixed prosthodontic .
