حساسية و نوعية اختبار المستضد السريع مقارنة بالزرع الجرثومي لمسحة البلعوم لتشخيص التهاب البلعوم بالعقدية A
Abstract
يبقى التهاب البلعوم و اللوزتين الحاد من أكثر الأسباب شيوعاً لمراجعة عيادة الأطفال، و يبقى إثبات التشخيص في منتهى الأهمية في حالات التهاب البلعوم بالمجموعة A من العقديات الحالة للدم لضرورة المعالجة بالصادات الحيوية وقايةً من الاختلاطات المحتملة، وتتوفر في الوقت الحاضر أعداد كبيرة من وسائل التشخيص التي تعتمد التقنيات المناعية، معظم هذه الوسائل تعطي حساسية و نوعية جيدة إضافة إلى سرعة الحصول على النتائج ولذلك تم اعتمادها لتشخيص التهاب البلعوم، وفي مقدمة هذه الوسائل اختبارالمستضد السريع.
تهدف الدراسة إلى تحديد حساسية و نوعية اختبار المستضد السريع مقارنة بالزرع الجرثومي لمسحة البلعوم لتشخيص التهاب البلعوم بالمكورة العقدية من المجموعة A .
أجريت الدراسة على (100طفل ) من مراجعي عيادة الأطفال بمشفى الأسد الجامعي ممن تتراوح أعمارهم بين (3-12) سنة خلال شهري نيسان و أيار من عام 2008 ممن لديهم أعراض التهاب بلعوم، حيث تم إجراء اختبار المستضد السريع إضافة إلى إرسال مسحة إلى المخبر لإجراء الزرع الجرثومي .
وتبين من خلال النتائج وجود حساسية و نوعية عالية لاختبار المستضد السريع حيث بلغت حساسية الاختبار (97.7%) و نوعيته (100%)، مما يجعل من اختبار المستضد السريع طريقة مختارة في التشخيص قبل العلاج .
Acute pharyngotonsilitis remains one of the most common causes of pediatric clinic visit, and it is very important to prove the diagnosis of pharyngitis with group A of hemolytic streptococci to determine the need for using the antibiotic to prevent the possible complication. Nowadays, there are many ways to diagnose pharyngitis depending on immuno techniques. Most of these ways have good sensitivity and specificity in addition to the quick results. That is why they have been adopted to diagnose pharyngitis and the best of them is the raped antigen test.
The study aims to determine sensitivity and specificity of rapid antigen test comparing with the bacterial culture of pharyngeal swab to diagnose pharyngitis with streptococci A. It involves 100 children (3-12 years old) with symptoms of Pharyngitis who visited the pediatric clinic patients of AL ASSAD UNIVERSITY HOSPITAL in April and May of 2008. Rapid antigen test has been done in addition to the bacterial cultureIn the laboratory, and results showed that rapid antigen test has high sensitivity (97.9%) and specificity (100%). This makes the rapid antigen test the chosen way to diagnose pharyngitis.
