التهاب الجيوب الفطري بالرشاشيات تشخيص ومعالجة
Abstract
لقد نوقشت المعالجة الفعالة لالتهاب الجيوب الفطري من خلال عرض لستة حالات سريرية، حيث تبين أن تشخيص التهاب الجيوب الفطري يمكن أن يغفل ما لم يكن هناك شكّ عالٍ بذلك عند التعاطي مع التهاب الجيوب المزمن,مع إجراء فحوصات نسيجيةٍ، ومخبريه ضرورية للتشخيص.
يصنف المرض إلى مرتشحٍ، وغير مرتشحٍ، وكل منها يقسم إلى قسمين, ركزنا في دراستنا على أهمية التشخيص النسيجي قبل العلاج الجراحي وعلى أهمية التصوير الطبقي المحوري قبل، وبعد العمل الجراحي,وكذلك أكدنا على أهمية المراقبة الطويلة الأمد للمرضى بعد المعالجة، وذلك لكشف النكس في المراحل المبكرة عن طريق تنظير الأنف، والجيوب.
We discuss the effective management of Para nasal sinus aspergillus's in six cases, which show us that fungal sinusitis may be easily missed unless high index of suspicion is maintained and specific culture and histology requested.
The disease is classified into invasive and noninvasive types, each of them being divided into two sub-groups. The importance of histological differential prior to selecting the appropriate treatment option is stressed. CT scan should precede definitive surgery and be used in follow up. Close and prolonged follow up is essential to treat any recurrence at early stage with intranasal endoscopies.
