التخدير حول الجافية بمشاركة الكلونيدين خبرة مشفى الأسد الجامعي باللاذقية ما بين 2001 - 2003
Abstract
يعتبر التخدير فوق الجافية أحد أشكال التخدير التي بدأت تأخذ مكاناً مرموقاً في الآونة الأخيرة وذلك لإمكانية استخدامه في مجالات عديدة.
هدف هذا البحث إلى دراسة التغيرات الهيموديناميكية في هذا النوع من التخدير عند استخدام مشاركات دوائية مختلفة واختيار أفضل المشاركات الدوائية التي تؤمن تسكينا جراحياً وما بعد جراحي جيداً بأقل ما يمكن من السمية الجهازية وقد أجريت هذه الدراسة على 45 مريضاً قسموا إلى 3 مجموعات:
1- مجموعة الليدوكائين + الفنتانيل 10 مرضى
2- مجموعة البوبيفاكائين + الفنتانيل 10 مرضى
3-مجموعة الليدوكائين + الكلونيدين + الفنتانيل 25 مريضاً
من خلال المقارنة وجدنا أن المجموعة الثالثة حققت استقراراً جيداً في الهيموديناميكية مع تسكين جراحي جيد إضافة للتسكين لفترة كافية في الفترة التالية للجراحة(1).
Peridural anaesthesia is a method which has recently occupied aconsiderable space in practice due to its several applications.
This study aims to evaluate the haemodynamic changes using different drug combinations on the one hand, and to take the best choice concerning the drugs which provide good surgical and post –surgical anaelgesia with a minimum systemic toxicity on the other hand.
This study has been performed 45 patients who divided to three subsets:
A-(Lidocaine+fentanyl) group 10 patients
b-(Bupivacaine+ fentanyl) group 10 patients
c-(Lidocaine +clonidine +fentanyl)group 25 patients
Comparisions showed a good haemodynamic stability,adeep surgical anaelgesia and a prolonged post –surgical anaelgesia with the third subset.
