تدبير بعض كتل العنق الخلقية في محافظة اللاذقية التي أجري لها عمل جراحي
Abstract
الكتل والنواسير الناشئة على حساب شذوذات في الأقواس الغلصمية في مدينة اللاذقية تعالج من قبل اختصاصات متعددة (الجراحة العامة، أطباء الأنف والأذن والحنجرة، أطباء الجراحة التجميلية). هناك الكثير من النقاط غير المتفق عليها بالإضافة إلى النسبة العالية من النكس بعد العمل الجراحي بشكل خاص الناسور الدرقي اللساني نتيجة الاستئصال المحدود لمجرى الناسور الممتد حتى قاعدة اللسان أضف إلى ذلك ترك قسم من الناسور خلف العظم اللامي، الكيسة الغلصمية ليس لها اتصال مع البلعوم ولا داعي إلى تسليخ خارج حدود الكيسة في حين الناسور الغلصمي له فوهة داخلية لا بد من استئصال الناسور مع المجرى حتى الفوهة في المسكن اللوزى مع استئصال اللوزة الكامل.
هذا المقال يقدم خبرة طويلة الأمد في معالجة هذه الشذوذات ويلخص الدروس المستقاة من الدراسة وتوضيح خطة الباحث في معالجة العديد من هذه الكتل والنواسير.
يخلص البحث إلى أن هناك حاجة إلى تدريب خاص لإجراء هذه الجراحات وذلك عن طريق اختصاص مستقل متفرع من الجراحة العامة أو الأذن والأنف والحنجرة.
Although anomalies of branchial apparatus and congenital neck masses in Lattakia have been treated by many specialties ,general surgeon, ENT, plastic surgeon ; certain points controversy still exit; particularly with respect to high rate of limited removal of the fistula tract witch extend to the base of the tongue, and leaving some of the fistula the body of hyoid bone, branchial cyst has no tract has no tract connecting it to the pharyngeal wall and no need to waist time during looking for it, how ever in second branchial fistula it is essential to trace the tract to upper tonsillar pole and remove the tonsil. This article provides review of long period of experience in dealing with this anomalies area of controversy are high lighted and clarified, conclusion drawn to the need for special training as separate specialty after basic general surgical training or training or E.N.T .
