داء البروسـيلات في اللاذقيـة دراسة للاختـلاطات والتشـخيص والعـلاج
Abstract
تمت دراسة 50 مريضاً مصاباً بداء البروسيلات تراوحت أعمارهم بين السنتين والـ 64 عاماً. لوحظت قصة تماس مع الحيوانات في ستة مرضى فقط مما يفترض أن العدوى حدثت في بقية المرضى عن طريق الحليب ومشتقاته. ركزت الدراسة على المظاهر التي تدل على توضع الإصابة (الاختلاطات) التي لوحظت في 18 حالة وكانت كما يلي: التهاب شغاف ثلاث حالات ، مظاهر عصبية ثلاث حالات (التهاب سحايا ودماغ حالتان وانضغاط نخاع حالة نجمت عن انهدام فقري) ، التهاب خصية وبربخ 3 حالات ، التهاب مفصل وحيد 4 مرضى ، والتهاب فقار 4 حالات ، واضطراب وظائف الكبد في حالتين.
تم التشخيص اعتماداً على إيجابية اختبارات التراص المصلي ، أو بمعايرة الأضدادIgM, IgG .عولج المرضى الكهول بـRifampicin معDoxycycline ،مع إضافة Streptomycin لدى إصابة العظام والمفاصل وعند النكس ، أما الأطفال الثلاثة فعولجوا بـ Cotrimoxazole
احتاج مريض واحد لتبديل الدسام الأبهري ، واستمر الضعف في الطرفين السفليين عند مريض واحد ، كما لوحظ انعدام النطاف عند مريض واحد ،وحدث نكسٌ تطلب إعادة العلاج في 4 مرضى ، وحدثت وفاة واحدة نجمت عن التهاب الشغاف على دسام صنعي ، وشفي بقية المرضى بشكل كامل.
نستنتج أن داء البروسيلات مرض جهازي ذو اختلاطات مهمة وخطيرة ، والسيطرة عليه ممكنة بمراقبة صناعة الألبان والأجبان ، والتأكد من معاملة الحليب وتعقيمه بالشكل المناسب.
50 patients with brucellosis were followed up over a period of 6 years. Ages varied from 2-64 years. Contact with domestic animals noticed in 6 patients only which suggests that the main source of infection is raw milk and dairy products.
Complications noticed in 18 patients, endocarditis (3), meningo- encephalitis (2) spinal cord compression (1), epididymoorchitis (3), monoarthritis (4), vertebral infection (4) and hepatic functional disorders (2).
Diagnosis was made by SAT (wright) or by Elisa titration of antibodies IgG, IgM. Adult patients were treated with Rifampicin plus Doxycycline. Streptomycin was added in bone or joint involvements and in relapsing cases, children were treated with Cotrimaxazole.
One patient required aortic valve replacement, paraplegia persisted in one patient with spinal cord compression. azospermia was noticed in one patient, 4 patients relapsed after initial treatment, one patient died due to endocarditis on artificial valve and the rest of the patients recovered fully.
Conclusion: Brucellosis is systemic disease with frequent and serious complicalions. prophylaxis depends on avoiding raw milk and good control of dairy production .
