موجودات التصوير المقطعي المتماسك في السمية الشبكية بالهيدروكسي كلوروكين
Abstract
Aim: to study SD-OCT findings in patients taking hydroxychloroquine , and to evaluate the possibility to detect retinal damage before appearance of clinical symptoms.
Methods: 15 asymptomatic patients who takes hydroxychloroquine as treatment for rheumatoid arthritis for 3 years at least and compare them with 15 of controls, examination was done using SD-OCT to measure retinal layers thickness, and compare imaging results with clinical examination.
Results: patients mean age was 54.17±11.23 years while controls mean age was 51.78±8.46 years. The difference was significant between patients and controls in central retinal thickness and parafoveal region ,whereas no significant difference was found in perifoveal thicknesor retinal nerve fiber layer thickness. And IS/OS damage was found in most patients.
Conclusion: hydroxychloroquine causes retinal injury which can be detected using SD-OCT prior to any apparent visual symptoms.
الهدف: دراسة موجودات فحص SD-OCT لدى المرضى الذين يتناولون هيدروكسي كلوروكين, وتقييم إمكانية كشفه للأذية الشبكية قبل أن تظهر الأعراض السريرية.
الطرائق: تمت دراسة 15 مريض غير عرضي ممن يتناولون هيدروكسي الكلوروكين في سياق علاج التهاب المفاصل الرثياني لمدة 3 سنوات أو أكثر ومقارنتهم مع 15 من الشواهد, تم الفحص باستخدام جهاز SD-OCT وتسجيل ثخانات الطبقات الشبكية , ومقارنة موجودات التصوير مع الفحص السريري
النتائج: بلغ وسطي أعمار المرضى 54.17± 11.23عاماً, في حين كان متوسط أعمار الشواهد 51.78± 8.46 عاما كان الفارق هاماً بين المرضى والشواهد في ثخانة الشبكية المركزية والمنطقة المجاورة للنقرة في حين لم يكن الفارق هاماً في ثخانة المنطقة حول النقرة وطبقة الألياف العصبية حول الحليمة, كما أظهرت طبقة IS/OS أذيات لدى معظم المرضى
الخلاصة: يسبب الهيدروكسي كلوروكين أذيات شبكية يمكن كشفها باستخدام SD-OCT باكراً قبل أن تظهر أي أعراض بصرية .
