تطبيق الزريعات التقويمية بالمشاركة مع التقشير السنخي في علاج حالات الصنف الثاني نموذج أول
Abstract
مقدمة: توضح هذه المقالة طريقة جديدة لمعالجة حالات الصنف الثاني / 1 بمشاركة التقشير السنخي الجراحي واستخدام الزريعات التقويمية ،إذ يتم إرجاع كتلة الأسنان الأمامية في مرحلة واحدة باستخدام قوة هيكلية (Orthopedic) اعتمادا" على الزريعات. المواد والطرق: تمت معالجة مريضين يشكوان من بروز في كتلة الأسنان الأمامية العلوية ، حيث تم قلع الضواحك الأولى العلوية وإجراء شقوق عظمية طولانية في العظم القشري من الناحيتين الدهليزية والحنكية ، بعدها تم إرجاع الأسنان الأمامية بقوة مقدارها 500غ/ جانب اعتمادا" على الزريعات وبعد إغلاق الفراغ أنهيت الحالة كالمعتاد. النتيجة: تم إرجاع كتلة الأسنان الأمامية خلال 4 أشهر للمريض الأول و3.5 شهر للمريض الثاني وحدث تحسن واضح في البروفيل الوجهي ووضعية الشفة العلوية.الاستنتاجات : يعدُّ التقشير السنخي وسيلة فعالة في اختصار مدة العلاج التقويمي ، وآمنة من حيث تأثيرها في حيوية اللب والنسج الداعمة ، ومشاركتها مع الزريعات يمكن أن يغني عن إجراء الجراحة التقويمية لمعالجة بعض حالات البروز .
Introduction: This article illustrates a new treatment system for class II/1 combining accelerated osteogenic orthodontics and osseointegrated mini-implants. Materials and methods: Two patients who complained of upper anterior segment protrusion were treated with this technique. Upper first premolars were extracted and the corticotomies were made on both the labial and palatal sides, then the retraction force of 500 gr/side was used to close the space. Result: The retraction period was 4 months for the first patient and 3.5 months for the second patient. Conclusions: Alveolar corticotomy is an effective method to decrease the orthodontic treatment time, and it has no side effects on pulp vitality or the periodontium and combining it with mini-implants can replace orthognathic surgery in some cases.
