The Association between Overweight and the Pathway of Delivery in Labor Women

Abstract

Background: High maternal body mass index (BMI) is associated with complications during pregnancy and delivery.

Aim: To study the association between body mass index and mode of delivery in full term pregnant Syrian women.

Materials and Methods: This observational case series study comprised of 130 women with at term labor with a gestational age between 37 – 41 weeks, admitted to Department of Obstetrics and Gynecology, Tishreen university Hospital, Lattakia, Syria, during the period of September 2019 to September 2020.

The Women were categorized into four different BMI classes according to pre-pregnancy (< 18 weeks) height and weight. Mode of delivery was categorized to vaginal delivery and cesarean section.

Results: Women according to mode of delivery were distributed into 87 (67 %) with normal vaginal delivery and 43 (33 %) with cesarean section.

Women delivered with cesarean section had significantly higher BMI than women delivered with normal vaginal delivery, 28.3 ± 6.4 kg/m2 versus 24.4 ± 3.5 kg/m2, respectively (P<0.001).

The rate of cesarean section was significantly higher in obese women followed by overweight women and finally by normal weight women, 51%, 30% and 20.8%, respectively.

Women who were obese had 4.01 times the odds of cesarean delivery compared to women with normal BMI (95% CI: 1.61–10) (P=0.0028).

Conclusion: Incidence of cesarean delivery increased with increasing maternal BMI. Obesity prior to pregnancy was associated with an increased odds of cesarean delivery.
