دراسة سريرية وتشريحية مرضية لاثنتي عشرة حالة ورم مخاطي كاذب مكتشفة صدفة في الصفاق بطريقة أخذ الخزعات بالإبرة (Tru cut needle biopsy)

Abstract

قمنا في هذا البحث بدراسة اثنتي عشرة حالة ورم مخاطي كاذب في الصفاق(بطريقة أخذ الخزعات بالإبرة Tru cut needle biopsy) شخصت سريرياً تحت عناوين أخرى مثل انتقالات خبيثة في الصفاق, كتلة على حساب الزائدة الدودية ، أو كتلة على حساب المبيض عند النساء. أظهرت الدراسة النسيجية وجود أشلاء لبشرة أسطوانية ذات طبقة واحدة ، أو ذات تعدد طبقات كاذب مع بحيرات من المخاط. تبين وجود تضارب شديد في الأدب الطبي حول طبيعة هذه الآفة (سليمة أو خبيثة) وحول كيفية التعامل معها عند اكتشافها، إضافة إلى صعوبات في تحديد الإنذار.

تفيد هذه الدراسة النسيجية الدقيقة في تحديد العناصر اللازمة للفصل بين الخبث والسلامة مع اعتبار أن السلامة هنا لا تعني إمكانية الشفاء بعد إجراء العمل الجراحي الذي لا بد أن يشرك بمعالجة كيميائية خاصة ، ووضع المريض تحت المراقبة الدقيقة ولفترة طويلة.

 

This is a study by Tru cut needle of twelve cases of Pseudomyxoma Peritonei misdiagnosed clinically as metastasis, appendicular or ovarian mass. The histological studies showed, in the different biopsies, columnar or pseudo stratified epithelium with uniform looking nuclei with exception in two cases, lakes of mucin, fibrous tissue with fibroblasts and chronic inflammatory infiltration. There is a great deal in the literature about the nature and source of the lesion, management and prognosis. This study will help in giving criteria to differentiate between benign and malignant lesions.
