Determining the Best Time Period in Treatment of Sudden Sensorineural Hearing Loss (SSHL)

Abstract

Background: The primary treatment for sudden idiopathic sensorineural hearing loss (SSNHL) is systemic steroids or intratympanic dexamethasone infiltration. The time from symptom onset to initiation of treatment is an important prognostic factor, but so far there is no clear time point for predicting treatment efficacy. There are few studies devoted to evaluating the effect of the time taken to start treatment on desired outcomes. Most studies conducted globally included patients who presented within less than seven days of the onset of symptoms and this falls within the recommended treatment time frame.

Objectives: The main objective: The research aims to determine the best time period in treating patients with sudden sensorineural hearing loss (SSNHL) by studying the effect of treatment start time on the results of improved hearing.

The secondary objective: To identify risk factors and comorbidities in patients who are visiting.

Materials and Methods: A descriptive statistical study (case series) that included 71 patients (39 females and 32 males) attending the Department of Otolaryngology in Tishreen University Hospital between 2020 and 2021 who suffer from sensorineural hearing loss matching the definition, aged between 17 and 70 years after the denial of other pathological causes. the patients were divided into 4 groups according to the time of starting treatment from the date of onset of symptoms:

Group I : Patients who visited during the first 10days of the onset of hearing loss.

Group II : Patients who visited  after the first 10days of the onset of symptoms, but within a period of less than a month.

Group III : Patients who visited within a period between (1-3) months from the onset of symptoms.

Group IV : Patients who visited after more than 3 months from the onset of symptoms.

- Systemic corticosteroid therapy was administered and improvement was evaluated according to the Siegel criteria.

Results:  We found that the improvement was the best during the first 10 days  of the onset of symptoms. as the percentage of patients who improved was (77.4%). and the improvement in most patients was a complete improvement. As for the patients of the second group. the percentage of improvement was about (58%). and the third group was About (46%) of them improved. while in the fourth group only (10%) of patients showed partial improvement on the audiogram. Aging, dizziness, fullness, viral infection and autoimmune diseases all indicated poor prognosis.

CONCLUSION: Initial treatment therapy should not be excluded in late-progressing patients who suffer from sudden sensorineural hearing loss (SSNHL).
